Mafia III: Definitive Edition is a remastered form of the 2016 open-world third-person action-adventure game, taking place in the fictional city of New Bordeaux, a lively recreation of 1968 New Orleans, and centres on Lincoln Clay, a veteran of the Vietnam War who survives a harsh betrayal at the hands of the Italian mob before launching a determined campaign of revenge to break up their empire bit by bit. Unlike its earlier titles, which relied on a linear, cinematic storytelling style, Mafia III uses a full open-world sandbox format in which the city is split into ten districts, each run by the Marcano crime family and its various underbosses. The main gameplay element is systematic liberation: Lincoln gathers information by questioning informants, bugging junction boxes, and destroying rackets—illegal activities such as drug dens, prostitution rings, and smuggling operations—which weakens the mob's control over each district.

When a district has been sufficiently destabilized, Lincoln faces and kills the racket boss before assigning the area to one of three underbosses (Cassandra, Burke, or Vito), each of whom gives the player special benefits and abilities but also becomes increasingly resentful if they are ignored, thus requiring a careful balance between loyalty and the distribution of power during the course of the game. The Definitive Edition improves on the original with enhanced textures, better lighting and visual effects, more refined character models, and greater audio fidelity, and it also includes all previously released downloadable content—such as the "Faster, Baby!" "Stones Unturned" and "Sign of the Times" story expansions—within the main narrative, giving players a more unified and complete experience from the beginning.

The combat in Mafia III: Definitive Edition is intense, rapid, and harsh, combining cover-based shooting with stealth features that let Lincoln eliminate enemies silently with his knife, carry out violent takedowns, and use tactical distractions such as throwing bottles or whistling to separate individuals from their groups. The gunplay includes a variety of period-correct weapons—from pistols and shotguns to assault rifles and grenade launchers—and features a combat sequence in which killing enemies one after another triggers slow-motion "Brutal Kills" that restore Lincoln's health and give the firefight a more cinematic feel.

Because of his military training, Lincoln has extra abilities such as drawing enemies in, marking targets through walls, and asking his three lieutenants for help; the latter can send supplies, vehicles, or even hit squads to assist in the fighting. Driving is a major part of the game, with a broad selection of 1960s-style vehicles that can be customised and upgraded, and the police respond to criminal activity based on a dynamic "heat" level that increases if Lincoln causes too much public disorder. The story is presented through a documentary-style framing device that includes interviews with surviving characters set years after the events, giving Lincoln's rampage a sense of historical significance and moral ambiguity.

The Definitive Edition comes with all the bonus weapons, vehicles, and clothing packs—for example, the "Family Kick-Back" and "Judge, Jury, and Executioner" packs—so that players have immediate access to extra tools of destruction right from the start. The main aim is to break up the Marcano crime family systematically, deal with the traitors who had left Lincoln to die, and determine the fate of New Bordeaux's criminal underworld—all while having to cope with the racial tensions and social unrest that existed in America in 1968, making Mafia III: Definitive Edition not only a revenge thriller but also a commentary on power and prejudice, all now featured with improved visuals and a full range of post-launch content.
